We had to visit a doctor while in Thailand for a minor stomach issue, and we followed CSA's directions very carefully. Upon returning to the US, we filed a claim for merely $100 and change--and they made us jump through the hoops about several dinky little things multiple times (e.g., the doctor had issued the itemized bills in both the local language and English, yet they asked for a translation, which, besides the fact it makes little sense in the first place, would have cost as much as the small amount claimed!). They asked for a couple of other documents, one after another. It would have been one thing to ask for all of those documents at one time, but CSA came after one item at a time, and it almost seemed as though they were buying time and hoping that we'd go away facing these roadblocks.
In the end, we just gave up. During the same trip, my luggage was damaged and I was planning on filing a claim on that as well, but I didn't bother after this experience. I guess there was a reason the coverage was so cheap. All in all, this was a horrible waste of my time and money. I have since taken my business elsewhere.
